Personal data

Personal data are all kinds of information about an identified or identifiable physical person, including in particular name, surname, address of residence or another place of stay, PESEL, e-mail address, data concerning health and other data describing physical, physiological, genetic, psychic, economic, cultural or social identity of a physical person.

Your personal data will be processed:

  • in health purposes connected with health services for you – including patient’s registration, maintaining and making available medical documentation – on the basis of art. 9 section 2 letter h of RODO and art. 6 section 1 letter c of RODO;
  • in order to execute the service agreement with the Administrator – on the basis of art. 6 section 1 letter b of RODO;
  • in order to protect against claims and to pursue claims as well as in order to ensure personal and property safety – on the basis of legally justified Administrator’s interest, according to art. 6 section 1 letter f and art. 9 section 2 letter f of RODO;
  • in marketing purposes and other purposes not mentioned under letters a and b – on the basis
    of your consent, according to art. 6 section 1 letter a of RODO;

Your statement of your personal data is voluntary but necessary for making use of services performed by the Administrators.

Your personal data may be transmitted to other subjects exclusively in justified cases and keeping confidentiality of the data. Your personal data may be transmitted to:

  • persons and other subjects performing health services by order of the Administrators (e.g. other medical subjects, laboratories, etc., in order to continue treatment, making ordered examinations/tests or granting other medical services);
  • the Minister of Health as the administrator of personal data processed in the Medical Information System within the Electronic Platform for Collecting, Analysis and Distribution of Digital Resources on Medical Events;
  • subjects performing hosting and tele-information services to the benefit of the Administrators
    (e.g. providers of software for maintaining of medical documentation and suppliers of server for keeping personal data);
  • subjects cooperating with the Administrators in the scope of maintaining and servicing visit booking systems;
  • persons authorised by you to get information about your state of health or access to the medical documentation;
  • insurance companies in case of raising claims against the Administrator;
  • subjects performing legal, bookkeeping and archiving services to the benefit of the Administrator;
  • public authorities and other authorities in the scope of their statute rights;
  • by collecting "cookies" - described below.

Types of cookies we use

Your personal data shall be transmitted neither to third countries nor to international organisations. The Administrators shall not apply automated decision taking in personal data matters.

Your personal data will be kept by the moment of prescription of claims for services granted to you.

The data included in the medical documentation will be kept by us for the period resulting from the general legal regulations in force.

In case of data processed on the basis of your consent, we shall process them by the time of your reporting objection against data processing, withdrawal of your consent for processing them or if the processing of these data becomes unnecessary for other reasons.

You have the right to access to personal data, their amendment, deletion or limitation of processing as well as the right to raise objection against processing and the right to transmit data.

The right to transmit personal data and the right to object against processing of personal data are not applicable to the data processed by virtue of art. 9 section 2 letter h of RODO, i.e. data processed for health purposes connected with granting you health services, including data contained in the medical documentation.

You have the right to appeal to the President of the Personal Data Protection Office.
